The library does not provide scantron forms.
If you're at the Oceanside Campus, you can pick up scantron forms from the administrative assistant assigned to your instructor's department. If you're not sure who that is, a librarian or library staff member can provide that information.
If you're at the San Elijo Campus, scantron forms are available from the administrative assistant in SAN 1141.
